How Guardian dental plans usually cover care
Guardian’s dental PPO network is commonly called DentalGuard Preferred, and Guardian describes it as a national network. Most Guardian dental plans come through employers, so coverage depends on the specific plan your employer chose.
Most dental PPO plans, from any carrier, share the same basic structure. The details below are typical, not a promise of what your plan pays — your employer or plan sets the exact rules, so check your plan.
| Type of care | Typical examples | How many PPO plans treat it |
|---|---|---|
| Preventive | Exams, cleanings, routine X-rays, fluoride, sealants | Often covered at the highest level, frequently without a deductible |
| Basic | Fillings, simple extractions, some gum treatment | Often covered at a middle level after the deductible |
| Major | Crowns, bridges, dentures, some root canals and implants | Often covered at a lower level, and may have a waiting period |
Emergencies and your Guardian plan
When a tooth breaks or starts to hurt, the first visit is usually a limited, problem-focused exam and an X-ray. Many plans cover that kind of exam under preventive or diagnostic care, but some count it toward the same limit as your regular checkups. If the tooth needs a filling, root canal or extraction, that is usually basic or major care with the deductible applied. We check what we can on the same day and explain your share before treatment.
Other typical rules: an annual maximum, frequency limits on cleanings and X-rays, and on some plans waiting periods for major care or a rule about teeth missing before the plan began. Check your plan for the exact details.
What we can do for Guardian members
Guardian members can come to us for emergencies and for everyday family care:
- Checkups and cleanings for the whole family, from age 1 through senior dentistry
- Kids’ care in our office: fluoride, sealants, and treatment of cavities with tooth-colored fillings and extractions
- Repairing and replacing teeth: root canals, crowns, bridges and dentures
- Tooth extractions, including wisdom teeth, done in our office
- Same-day emergency visits during office hours — call first
- Saturday appointments, 9am–3pm
Estimating your cost
Your cost with a Guardian PPO plan depends on your deductible, the coverage level for each type of care and your remaining annual maximum.
Before treatment, you get a written estimate of what your plan is expected to pay and your share. Your plan makes the final decision when the claim is processed.
Without insurance, new patient visits are $49, $99 or $179. Root canals start at $899, crowns at $1,199, 3-unit bridges at $3,499 and dentures at $1,399 (per arch for full dentures). See dental prices. If Guardian pays part and you owe the rest, you can spread the balance with Cherry, Sunbit or CareCredit (see financing options), or use an FSA or HSA card for dental treatment.
When to call us
Call us the same day if you have a toothache, a broken or knocked-out tooth, or swelling — during office hours, call first so we can fit you in. Go to the ER or call 911 for swelling that spreads to your eye or neck, trouble breathing or swallowing, or a high fever. Call (832) 612-2831 — we’re open Tuesday, Wednesday and Saturday, 9am–3pm.
